The approved or notified operation of an X-ray facility has been or will be terminated. The notification is made by you as the person responsible for radiation protection, authorized representative or radiation protection officer.

- Complete the form provided.
- Depending on the case, upload proof of disposal, whereabouts or inoperability of the X-ray equipment.
